FEES & INSURANCE

Root to Bloom Nutrition Therapy accepts insurance with Blue Cross Blue Shield.

  • You’ll complete our insurance form & script before our first session to check your nutrition therapy benefits and coverage.

    Please note that Cherry Blossom Nutrition RDN PC is a medical provider and is currently only contracted with Blue Cross Blue Shield. If you have a different insurance, you can still ask about your out-of-network benefits.

  • If you are contracted with another insurance company, you might be reimbursed for seeing an out-of-network provider. This means we are not contracted with your insurance directly, but most (not all) plans will allow you to submit a Superbill (a detailed receipt) for reimbursement. This means you are responsible for payment at the time of service, and then you will work with your insurance company directly to secure reimbursement.

    Before our first session, you can complete our insurance form & script to check what your out-of-network benefits are.

  • If you don’t have insurance or your insurance doesn’t include out-of-network coverage, you may self-pay for services. We provide 2 options for self-pay: regular self-pay and Equity Pricing self-pay.

    Root to Bloom Nutrition Therapy is rooted in values of inclusivity and accessibility. We understand many circumstances can affect your ability to access care, like:

    If you identify with this/these experience(s) and feel that the Equity Pricing option will provide greater access to our services that would otherwise not be accessible, we invite you to choose the equity pricing option!

  • Yes! All major credit cards, including HSA and FSA cards, are accepted forms of payment.

  • Refunds will not be issued for services already conducted. If, for any reason, we are unable to fulfil the service, you will be refunded in full for any part of the services paid for but not yet rendered.

  • A 24 hour notice is required for canceling appointments. Appointments canceled within 24 hours of the appointment time will be billed a $60 late fee. The late fee will be waived in cases of emergencies or extenuating circumstances at the discretion of Root to Bloom Nutrition Therapy.

    A 48 hour notice is required for rescheduling appointments. If the client reschedules with less than a 48 hour notice and successfully reschedules their appointment time within a week of their original appointment date, the late fee will be waived.

Self-Pay: $180 | Equity Pricing: $140
